Standard IVF with ICSI

Standart IVF + ICSI (with own eggs)

Intracytoplasmic Sperm Injection (ICSI) is a technique used in the IVF process to address challenges related to sperm quality. In standard IVF, sperm and eggs combine naturally in the laboratory. However, with ICSI, a single sperm is selected and injected directly into the egg. This approach helps when sperm have difficulty reaching or penetrating the egg on their own.

ICSI is beneficial for those with suboptimal sperm quality and can also be combined with advanced Preimplantation Genetic Diagnosis (PGD) or used when sperm fragmentation is high. After fertilizing the egg using the ICSI technique, the embryo is monitored and nurtured until it reaches the blastocyst stage on day five. At this point, it is ready for implantation, freezing, or further genetic testing.

Standard IVF with ICSI (intracytoplasmic sperm injection) is a modern infertility treatment method that combines traditional IVF with ICSI technology. This approach significantly increases the chances of successful fertilization, especially in cases where there are problems with sperm quality or quantity.

What is ICSI?

ICSI (Intracytoplasmic Sperm Injection) is a procedure in which a single sperm cell is directly injected into the cytoplasm of an egg using specialized micromanipulation techniques. This method helps overcome challenges related to low sperm motility or abnormal morphology, ensuring successful fertilization even in cases of severe male infertility.

Indications for IVF with ICSI

IVF with ICSI is recommended in the following cases:

  • Male factor infertility, including low sperm concentration (oligozoospermia), reduced motility (asthenozoospermia), or a high percentage of abnormal sperm forms (teratozoospermia).
  • Unsuccessful conventional IVF attempts, when previous IVF cycles have not resulted in successful fertilization.
  • Presence of antisperm antibodies, where immune factors prevent natural fertilization.
  • Use of cryopreserved eggs or sperm, to enhance the chances of success after thawing reproductive material.

Stages of IVF with ICSI

Standard IVF with ICSI is carried out in several stages:

  1. Preparation and Examination
At this stage, a consultation with a reproductive specialist is conducted. Health assessments are performed, including the collection of the medical history and determination of the indications for IVF with ICSI. Necessary laboratory and instrumental tests are also performed, such as blood tests, sperm analysis, and ultrasound examination of the female reproductive organs.
  2. Ovarian Stimulation
After a comprehensive health assessment, hormonal medications are prescribed to stimulate the ovaries in order to obtain multiple mature eggs. Follicular development is monitored with regular ultrasound exams and blood hormone level tests.
  3. Follicle Aspiration and Egg Retrieval
This procedure is performed under anesthesia, as transvaginal follicle aspiration to retrieve mature oocytes is not painless.
  4. Sperm Preparation and ICSI
At this stage, the sperm is processed in the laboratory to select the highest-quality sperm. Then, a single sperm is injected directly into the egg using specialized micromanipulators under a powerful microscope by a qualified embryologist.
  5. Embryo Culture and Embryo Transfer
The embryos are cultured under close monitoring by specialists for 3-5 days in the laboratory under optimal conditions. If everything progresses successfully, the embryos are transferred into the woman’s uterus. The highest-quality embryo is selected to improve the chances of successful implantation. The embryo transfer procedure is painless and does not require anesthesia.
  6. Luteal Phase Support and Waiting for Results
After embryo transfer, the woman is advised to take progesterone medications to support the endometrium and promote embryo implantation. The final stage of IVF with ICSI is a pregnancy test, which is performed 10-14 days after the embryo transfer.

Possible Risks During IVF with ICSI

Like any medical procedure, IVF with ICSI carries certain risks. Potential complications include:

  1. Ovarian Hyperstimulation Syndrome (OHSS)
This can occur in response to ovarian stimulation, but with careful monitoring, the risk is minimized.
  2. Multiple Pregnancy
The likelihood of multiple pregnancies increases if more than one embryo is transferred. However, doctors usually recommend transferring only one high-quality embryo to reduce this risk.
  3. Risks Associated with Aspiration Procedure
There may be minor complications during the follicle aspiration procedure, but it is generally a minimally invasive and safe procedure.

Standard IVF with ICSI is a reliable method of infertility treatment that significantly increases the chances of successful conception, especially in cases of male factor infertility or previous unsuccessful IVF attempts.
